Status assessment

Before your dental treatment starts, your dental problem has to be identified. The dentist has to collect data and information using a digital panorama x-ray, an oral camera, dental mirrors and other hand tools and asking questions from you. By analysing the information your dental problem is identified.

Digital panoramic X-ray

Is a very efficient diagnostic tool used during the status assessment. It combines the advantage of panorama and digital X-ray techniques.

Panoramic X-ray, also known as orthopantomogram or panorex, is a panoramic scanning dental X-ray of the upper and lower jaws. It shows a two-dimensional view of a half-circle from ear to ear.

The digital X-ray is a form of X-ray imaging, where digital X-ray sensors are used instead of a traditional photographic film. Advantages include:

Our Clinic has the most up-to-date, Siemens and German made digital panoramic X-ray equipment. You can see the image of the digital panoramic X-ray on a LCD display fitted to each dental chair.

Oral camera

Is an efficient examination tool used during the status assessment. It substitutes the dental mirror traditionally most widely used by dentists.

A high resolution digital micro camera is placed into your mouth, and on a colour screen the mouth and teeth can be seen. With it much better visualization and diagnosis can be made than with a traditional dental mirror.

The benefits of the oral camera are:

Annual control examination

During the guarantee period you have to visit the Clinic once a year to check your dental status.

Why status assessment is necessary? For instance: if somebody does not keep oral hygiene right it can badly effect the result of the treatment. In time observation and intervention the problem can be stopped and reserved.

After the guarantee period annual control is recommended.

Dental implants

A dental implant is an artificial tooth replacing missing tooth.

Implants can fully replace your bridges or denture or can more efficiently support your bridge or denture than any other methods. You can either replace all missing teeth with dental implants, or replace them partially with implants, which can support bridges or denture.

A dental implant consists of a titanium rod, which is placed into the jawbone. The titanium rod needs to be boned (gone through the osseointegration) with the jawbone which can take from 3 to 6 months. During the bone integration (osseointegration) you will get a temporary crown. After bone integration a false (artificial) tooth, which is either a crown, bridges or a denture is applied to the implant (titanium rod).

Implantation in steps

1st trip. Status assessment and extraction

If your are in good health and dental condition and your jawbone is strong enough you may need only 1 trip with at least 5 days stay. In this case you will have new teeth within a week instead of 2 or 3 trips. Contact us

During the 1st trip you will need to have a dental examination to determine whether the dental implant is the best option for you. You will have a panorama digital x-ray and several tests to make sure that the jawbone is healthy enough to carry the dental implant. If everything is fine the dentist will extract the tooth. After tooth extraction you will have to wait between 2 to 3 months for the gum to heal properly. For the healing period you will get a temporary tooth. You will need to stay for at least 1 day.

Missing tooth

If you had tooth extraction at least 4 to 10 weeks earlier or you have missing teeth, there is no tooth extraction needed and the implantation can start during your 1st visit. In this case only two trips are required.

If your jawbone is not strong enough you will need a bone substitution during your 1st visit. After bone substitution you will have to wait at least 6 months for the bone integration of the substituted bone.

2nd trip. Implantation and temporary crown

During the 2nd trip the dentist will set the implant (the titanium rod) and you will get a temporary crown. This requires a small operation when the dentist makes incision in the gum in order to gain access to the jawbone. This can be carried out under anaesthesia (sedation) and you will not feel any pain during the operation. The bone is prepared and the implant is inserted into the jawbone with high precision. Finally, the dentist stitches the gums, prescribes medication if necessary, and prepares the temporary crown. You may feel some discomfort during the week following the surgery. This is usually due to having stitches and the normal healing process. The implant needs to be integrated (osseointegration) with the bone after it has been placed, which takes at least 3-4 months in the lower jaw and 4-6 months in the upper jaw. You will need to stay at least 1 day.

Placing the implant
into the jawbone

3rd trip. Get the final crown, bridge or denture

After the implant has gone through the bone integration period (osseointegration), which lasts at least from 3 to 6 months, you need to return to the clinic to place the final crown, bridge or denture to the dental implant. On the 1st day the dentist will assess the bone integration and make the teeth impressions, which are taken to our dental laboratory to manufacture the final precision made crown, bridge or denture. On the 5th day, when the final crow, crown, bridge or denture are ready, you have to visit the clinic to place them on your dental implants. You will need to stay for at least 5 days.

Limitations

The primary limiting factor in implant therapy is the amount of bone available to support the implant. There is a number of tests to assess the amount of bone. If a patient does not have enough bone to support an implant, bone substitution (bone grafting) may solve that problem.

A crown is a cap placed over a carefully prepared natural or implanted tooth. The shade of porcelain match your surrounding teeth, making the crown appear as natural as your existing teeth.

Once the crown is permanently cemented, your chewing ability is like that of your natural teeth and you may again enjoy your natural appearance.

A crown can be either porcelain fused to metal or gold, porcelain fused to titanium, zirconium_oxide, porcelain jacket or temporary crown

If you have a missing tooth we can replaced it with bridge, dental implant or dentures.

Bridge

You can replace one tooth or multiple missing teeth with a bridge. A bridge permanently connects two existing (natural or implanted) teeth, filling in the area left by a missing tooth or teeth. Two ends of the bridge is anchored to natural or implanted teeth. The materials used for creating a bridge allow a custom shade of porcelain to match your surrounding teeth.

The crowns hold the bridge and the existing (natural or implanted) teeth, on each side of the missing one(s), hold the crown. The teeth on both sides of the missing one(s) must be prepared as crowns, which will serve as abutments to hold the replacement teeth in place.

If you have more than three missing teeth next to each other we can replace them with dental implants which will hold the bridge.

A bridge can be either porcelain fused to metal or gold, porcelain fused to titanium or zirconium oxide

Dentures

If you have several missing teeth next to each other denture can be a solution to replace your missing teeth. Dentures have been popular and millions have them.

You can choose from a wide range of high precision denture solutions if you replace some of your missing teeth with dental implants. Without implants the old type denture has several limitations (see Advantage and limitations of dentures) compared to the opportunities provided by dental implants.

Before dental implants, there was no solution to fix a full denture. Today you can replace some or all missing teeth with dental implants. Dental implants are suitable to anchor and fix your denture or replace the full arch with a bridge or implants. With implants you can enjoy a lot of advantages permanent, stable, high aesthetic denture which prevents bone loss.

Do you wear a denture currently? We can rework to make it suitable for implanted teeth. Implants will make your denture more stable, convenient and aesthetic and prevents bone loss. With a reworked denture you can save money without compromising the quality. During the status assessment our dentist will look at your current denture to advice if it is suitable for rework.

A conventional denture is removable. There are permanent solutions, which are bonding or clipping onto dental implants. See Solutions with implants

If all of your teeth are missing on the upper or the lower jaw, you will need a full denture or dental implants. If not all of your teeth are missing partial denture or implants are the solutions. Partial dentures are held in place by attaching them to the remaining natural teeth or dental implants. Metal claps are the most common type of attachment, but they cause problems: they are visible and not aesthetic, they loosen easily, and teeth can easily decay under them. Precision attachment or dental implants solve these problems.

An upper denture replaces missing teeth on the upper jaw (maxillary arch), and a lower denture on the lower jaw (mandibular arch). You may need an upper or a lower denture, we have a full spectrum of denture solutions for both.

To the top

Precision attachment

There are no clasps (metal hooks) and no metal is showing while you are wearing a partial denture with precision attachment. The result is a natural smile with no visible evidence that you are wearing a denture. Despite it is a removable denture, the precision attachment partial denture is stable and comfortable, remaining firmly in place no matter what food is eaten, while no interference with speech. They will not cause decay.

The teeth to which the precision attachments are connected must be capped (crown) since that is how it is fixed.

Precision attachment

Structure and materials

The base of a denture is a plate and can be made of either metal or acrylic (plastic). The teeth are made of hard acrylic or porcelain and match your natural teeth.

Advantage and limitations of dentures

Traditional old fashioned dentures can provide good aesthetics and function in a relatively cheap way, however with limitations. They can affect the taste, occasionally slip when you laugh, cough or smile, may break if dropped, pronouncing certain words may require practice, you may feel uncomfortable with new for a few weeks. You have to keep oral hygiene at high level every day and you have to go for regular inspections. You can eliminate these limitations by having dental implants.

Ill-fitting dentures are often a result of an inadequate lower ridge. After missing your teeth the jawbone begins to shrink. This process is called resorbtion. The lower jawbone shrinks around 3-4 times faster than the upper jawbone. Lower dentures become ill-fitted causing irritation, pain, discomfort, difficult chewing, slip when you eat or speak and so they need regularly refitting due to resorbtion. Dentures combined with dental implants solve loose denture problems and will even preserve bone in the jaw.

To make your denture(s) you need 1 trip and 5 days stay. See Length of treatments

Our guarantee for dentures are 2 years. See Guarantee

Inlays (porcelain, empress, gold)

You can get inlay and onlay when composite fillings would not be strong enough over a larger surface area and your tooth is good enough to support inlay instead of a crown. Inlays and onlays offer better strength and durability than fillings, and cost less than crowns.

An inlay consists of a solid substance (porcelain, empress or gold) fitted to a cavity and cemented into your tooth. An inlay is placed on the chewing surface between the cusps of the tooth, while an onlay covers one or more of the cusps.

An inlay can be made from porcelain, empress or gold. Empress is a mixture of around 80% porcelain and 20% plastic. The plastic makes it flexible and due to its sideline better fits to the cavity than porcelain. Gold inlay fits the best to sideline of the cavity and lasts the longest.

Composite fillings

You can get composite fillings when a non significant portion of your tooth has been lost due to decay or an accident.

Composite fillings are more attractive than silver amalgam and do not contain mercury as amalgam fillings. Composite fillings are made from resin which is moulded to the shape of the tooth. The filling material composed of plastic with small glass or ceramic particles, and hardened with a special light. The shade of the filling match your surrounding teeth making the filling appears as natural as your existing teeth.

Amalgam removal

Many clients are now choosing to have their black amalgam fillings removed due to health and aesthetic reasons. Most people have had amalgam fillings, which contain mercury and not aesthetics.

You may also want to remove amalgam filling and replacing it with an inlays or composite fillings, which are safe and very aesthetics. Inlays a better choice when the fillings are too large for composite fillings.

We ensure your safety during amalgam removal. Amalgam fillings contain mercury and we follow a special protocol to protect you, our staff and the environment. We collect the removed amalgam in a special container and we handle it as contaminated material.

Root canal treatment

The root connects the tooth to the jaw, see tooth structure. There is the pulp inside the root. Root canal treatment removes the pulp of the root and fills it with an inert material and seals it.

When the pulp is diseased or injured, the pulp tissue dies and has to be removed. A tooth without a pulp is like a tree, which runs dry but still stands and resists external impacts but becomes weaker and weaker. If you do not remove the pulp in time, your tooth gets infected and this causes intense pain and can lead to a dental abscess. After the dentist removes the pulp, the root canal is cleaned and sterilised, filled and sealed off to protect it. After a root canal treatment a crown, inlay, bridge, filling or veneer is placed over your tooth to make it stronger, functional and looks like as a natural tooth.

Occasionally a root canal treatment alone will not be enough to remove abscess and relieve pain and the end of the tooth, called the apex (tip or end of the root), has to be removed by an interaction called resection.

Whitening (bleaching) and prevention

BEYOND Whitening Accelerator

Bacterial pigments, foodstuffs, tobacco or medication can stain teeth typically. As a person ages the teeth often become darker. An effective bleaching can produce dramatic improvements in the cosmetic appearance of most stained teeth.

Beyond Whitening Accelerator removes stains on both surface and deep inside the dentin of your teeth, restoring them to beautiful white. The whitening makes your teeth whiter, glossier and shinier. Beyond Whitening Accelerator technology can wipe away years of smoking, coffee drinking or aging within an hour, and has even proved effective in fighting tetracycline stains. The whitening is comfortable, safe and effective.

Anaesthesia (sedation)

Anaesthesia makes loss the entire or part of the body sensation with or without affecting your consciousness in order to eliminate pain.

Local anaesthesia eliminates the pain in the teeth and the region of the treatment without affecting your consciousness. On the contrary sedation under General anaesthesia affects your consciousness. At our clinic we use intravenous General anaesthesia and Local anaesthesia.

If you would like to have dental treatment without remembering your treatment in a calm, safe, and peaceful way, we recommend you General anaesthesia

Local anaesthesia

Local anaesthesia eliminates the pain in the teeth and the region of the treatment without affecting your consciousness.

Our dentists determine very carefully the dose of the anaesthetic material to be injected and the place of the injection in order to fully disclose the possibility of overdosing which can cause gum problems later.

You may feel a little discomfort during injection, that we can eliminate by spraying anaesthetic material to the place of the injection.

General anaesthesia

You will be deeply relaxed. After sedation you will not have post fear and anxiety, you will not remember the treatment, sounds and smells.

We combine General anaesthesia with Local anaesthesia in order to set up level of sedation to (iii) responsive to voice only (see Note below). When you are on the level of (iii) responsive to voice only you are calm, relaxed and can answer easy questions and after sedation you will not remember the treatment.

Our anaesthesiologist Dr. Laszlo Zubek carefully monitors you throughout the procedure of sedation to ensure your safety. He continually monitors your blood pressure, pulse, oxygen levels in your blood and your heart during general anaesthesia. This is a standard, and considered a must at our clinic. He will make sure the level of your sedation remains on (iii) responsive to voice only during the treatment.

After finishing the treatment, you will stay in the dental chair until you fully wake up, and then we take you to your hotel to have a rest for the day. We ask you to drink plenty of water after sedation.

At our clinic we use Midazolam, Ditrivan and in some instances Propofol. These drugs are very safe and provide excellent dream-like states.

Note: level of sedation can be measured on a 6 level scales: (i) agitation, (ii) calm, (iii) responsive to voice only, (iv) responsive to shaking only, (v) responsive to pain only, and (vi) not responsive. We set up level of sedation to (iii) responsive to voice only.

Extraction

Extraction is the removal of a tooth from the mouth. Extraction can be simple or surgical extraction. Simple extractions are performed on teeth that are visible in the mouth, under local anaesthetic, and require an instruments to elevate and grasp the visible portion of the tooth.

The most common reasons for extracting a tooth are:

General anaesthesia (sedation) or local anaesthesia is provided for your comfort. That means you will have minimum discomfort and anxiety. Surgical extraction

When the dentist can not easily access your tooth for simple extraction you may need surgical extraction. The two main reasons are either because the tooth has broken under the gum line or because it has not erupted fully. The dentist may elevate the soft tissues covering the tooth and bone and may also remove some of the overlying or surrounding bone tissue.

Wisdom tooth extraction is usually necessary if you do not have sufficient space left to grow for the wisdom teeth in your mouth. There are four wisdom teeth, one at each of the four ends of the mouth. They are in the last in your teeth growth sequence and that is why may not sufficient space to left in your mouth by other teeth. This causes a number of wisdom tooth problems.

Because of not having sufficient space left to grow, a wisdom tooth can grow sideways or become trapped under the gums. When it partially erupts into the mouth, it can form a packet that traps food. The trapped food can cause tooth decay and the second molar in front of the wisdom tooth may decay. If the gums become infected, it can cause swelling, pain, headache and a bad odour. If the infection is left untreated it can continue around the wisdom tooth and cause facial swelling that can prevent the mouth from opening.

The pressure from a wisdom tooth trying to get space into the mouth can cause the rest of the teeth to move out of place. Early wisdom tooth surgery may even avoid the need for orthodontic treatment. When a wisdom tooth is badly trapped inside the jawbone, a cyst may form around the wisdom tooth.

A wisdom tooth problem can interfere with your life. Pilots and submariners often have all their wisdom teeth removed to avoid any potential problems that may interfere with their duties. If you travel or have a very busy period, an infected wisdom tooth can disrupt your time.

When a wisdom tooth symptom occurs as described above, it is important to have your wisdom tooth removed.

Resection

Occasionally a root canal treatment alone will not be enough to remove abscess and relieve pain at the end of the tooth, called the apex (tip or end of the root). In such case the apex has to be removed by entering through the gum and surgically removing the diseased material.

The treatment requires 1 trip and 1 day stay. See Length of treatments

Bone substitution (bone grafting)

If a patient does not have enough bone to support an implant, bone substitution may solve that problem.

We use synthetic and natural material as bone substitution. Natural substitution is taken from the patient's own jawbone where the density of bone is high enough. The natural material is mixed with the synthetic substitute and that mix is carefully placed to the area of the missing bone.

After bone substitution, you will have to wait at least 6 months for the bone integration of the substituted bone. See Dental implants

Our orthodontic specialist is Dr. Gabor Hermann. We use the latest international orthodontic methods of today:

During your 1st visit we listen to your reasons for coming and about your expectations. Then we make a dental and a facial examination. In many cases there are several facial and smile-aesthetic changes, that are possible to be solved in a way you might not think. We provide more treatment alternatives based on your expectations. Besides this, we introduce the possibilities in treatment given by different brace appliances. During the consultation we show examples of different braces, we also illustrate before and after pictures of our cases, using animations and special demonstration programmes. After the first consultation the inquirer may wish to pose further questions or ask for more information.

The next step to the treatment is preparing the treatment plan, final quotation.

The treatment In a significant amount of cases there is a necessity to treat the lower and upper denture at the same time. In most cases the expectations of patient require full orthodontics treatment. Sometimes the expectation is to have a minor intervention, but due to the state of the teeth, it is not possible and the desired goal can only be achieved with a full treatment.

The treatment of the upper and lower denture provides a wide range of possibilities. With a full treatment we can recognise a rank of teeth, adjust an ideal alliance between the upper and lower denture. Also on the issue of facial and smile aesthetics almost all of the objectives can be achieved.

A full treatment can be with fixed or non-fixed braces and can be visible, discreet or invisible.

Periodontics

Periodontics treats periodontitis, an inflammation of tissues that surrounds and supports the teeth or implants in the mouth.

Periodontitis is a very common disease affecting approximately 50% of adults over the age of 30 years. If periodontitis is left untreated it may lead to tooth loss in around 35% cases.

Patients should realize that the gingival inflammation and bone destruction are largely painless. Hence, people may wrongly assume that painless bleeding after teeth cleaning is insignificant, although this may be a symptom of progressing periodontitis.

Symptoms may include the following:

Treatment:

The stabilization of your periodontal state largely depend on your oral hygiene. Without daily oral hygiene, periodontal disease will not be overcome, especially if you have a history of periodontal disease.

When you visit us 1st you will undergo a Dental hygienic treatment. In advanced cases, there might be a need for periodontal surgery in order to have better access for cleaning the root surfaces and creating better anatomy of the bone and the gums. Local anaesthesia or sedation is offered to avoid any discomfort whatsoever. During the treatment you learn exactly what periodontal disease you suffer from. One very important aspect of the treatment is that you will learn how to perform careful home care, which will prevent you further problems with your teeth and gums.
